The forms on this page are used by current students, applicants and parents to request or disburse financial aid from the college. Most forms are generic (non-aid-year specific) but must be completed for each aid year for which you are applying (aid years run from summer through spring quarters). However, some forms are aid year specific and are noted as such. If you are unsure as to what forms to complete, check your specific requirements under My Info on MySCAD or contact the appropriate person for assistance. SCAD Federal/State Financial Aid Application (PDF -- 91K)
This form is required annually for all students applying for aid.

SCAD Financial Aid Application for Federal Direct Parent Loan for Undergraduate Students ( Parent PLUS) (PDF -- 93K)
This form is required only for parents of dependent undergraduate students who wish to borrow a PLUS loan. Get complete details on this loan.

SCAD Financial Aid Application for Federal Direct PLUS for Graduate Students (Grad PLUS) (PDF--87K)
This form is required only for graduate-level students who wish to borrow a Graduate PLUS loan. on this loan.

Verification worksheets

Required if you are a qualifying Georgia resident undergraduate student to apply for funds from the State of Georgia. It is important to apply at the same time for all funds you believe you are eligible for. Failure to do so will delay processing of state aid funds. The Financial Aid office will not accept any paper applications for Georgia aid; they must be mailed directly to the .

Georgia Residency Verification Form (PDF -- 80K)
This form is required in addition to the e-HOPE/e-TEG Application to apply for funds from the State of Georgia if you are not applying for federal aid (completing a FAFSA). This form also may be required when additional information is needed to determine residency for Georgia state aid programs.

Request for Change in Financial Aid Form (PDF -- 94K)
Use to request changes in existing financial aid only.

Parent Request for Student to Receive Additional Loan (PDF -- 97K)
Parent borrowers denied a PLUS loan due to adverse credit may use this to request additional federal student loan funds.
